Kids VBS Registration Form — Vintage Plane Teal Shield

Register every child with one bright, complete form in the Vintage Plane Teal Shield design! Our team built this registration form to capture everything a VBS leader needs at check-in — and the photo permission answer is right on the same page.

The form includes Child's Name, Date of Birth, Age, Grade, Parent / Guardian, Phone, Email, Address, Allergies / Medical Needs, Additional Support Needs, Authorized Pickup Persons, and a Photo Permission section (Yes / No) asking whether the child may appear in church VBS photos.

WHAT YOU WILL RECEIVE
This is a digital download (1 ZIP file) with this form in ONE print size — 8.5 x 11 inches (US Letter) — as both a PDF and a PNG:
- 8.5 x 11 inches — 2550 x 3300 px — 300 DPI
This form is letter-size only, sized to fit a standard sheet of paper with nothing cut off. Print as many copies as your church needs, at home, at a local print shop, or through an online print service.

PLEASE NOTE
- No physical item will be shipped — your files are ready to download right after checkout.
- This listing is for the single form design shown. A matching VBS Volunteer Sign Up Sheet in this same design is also available in our shop — collect the pair for a coordinated VBS check-in table.
- Colors may vary slightly from screen to print.

At Bright Path Ministry, every resource starts with an original idea from our leadership team. We bring those ideas to life with the help of AI image tools, then our team reviews and prepares each one with care — so you receive something made on purpose for you.
